Abstract Background In cases of papillary thyroid carcinoma (PTC), lymph nodes are a typical site metastasis. Extrathyroidal tissue involvement in differentiated causes the disease to progress and affects patients’ treatment options. Papillary (PTC) is common type (DTC), other follicular (FTC). Ectopic sites, such as intrathyroidal node invasion classical malignancy, extremely uncommon. It can be difficult detect an (ITLN) during histopathological test, but it important do so since stage subsequent treatment. Case presentation We present case 63-year-old woman who presented with multinodular goitre underwent total thyroidectomy. She was diagnosed aberrant metastases pathological testing, resulting upgrade Owing exceptionally unusual ectopic position metastasis, this case, there risk diagnostic therapeutic misinterpretation. This patient treated high-dose radioactive iodine view metastases. Conclusions Identification ITLN by imaging characteristics sometimes difficult, being often intraoperative discovery correlation that lead upstage alter final management.
